THE SCIENCE
What Is Platelet-Rich Plasma (PRP) Therapy?
Platelet-rich plasma (PRP) therapy uses your body's own biology to stimulate repair and regeneration at a targeted site. A small sample of your blood is drawn and processed through a centrifuge to concentrate platelets at 5 to 10 times their normal level in the blood. This concentrated plasma — rich in growth factors including PDGF, VEGF, TGF-β, EGF and IGF — is then injected precisely at the treatment site to stimulate vascular repair, nerve regeneration and tissue health.
At Shockwave Revibe Clinic, PRP is used primarily in the context of sexual health and erectile function. The therapy is applied to the corpus cavernosum, where it stimulates the repair mechanisms underlying erectile tissue health — the same vascular and nerve structures targeted by focal shockwave therapy. Because PRP uses the patient's own blood, there is no risk of foreign body reaction or rejection.
The evidence base for PRP in erectile dysfunction is at an earlier stage than that for shockwave therapy — current evidence includes case series and early RCTs (Matz EL et al., J Sex Med 2018; Raveenthiran S et al., Andrologia 2022) reporting improvements in IIEF-5 scores and patient satisfaction. The most compelling clinical rationale is for combination therapy: focal shockwave primes the tissue environment, and PRP delivers concentrated growth factors into biologically activated tissue — a synergistic approach supported by strong biological logic and emerging clinical evidence. (Dr Bahl, Chapter 11)
Dr Kishore Bahl — Shockwave Therapy in Clinical Practice, 2026. Chapter 11.
"Shockwave therapy primes the vascular environment; PRP amplifies the growth factor signal. The biological rationale for combination therapy is compelling; clinical evidence for the combination is emerging."— Dr Kishore Bahl, Shockwave Therapy in Clinical Practice, 2026
HOW IT WORKS
How PRP Therapy Works
1 · Blood draw & preparation
A small volume of blood is drawn from your arm — typically 20–30ml. The sample is placed in a centrifuge and spun to separate and concentrate the platelet-rich plasma from other blood components. The quality of preparation — platelet concentration, activation method, growth factor content — varies significantly between systems. At Shockwave Revibe, clinical oversight of preparation is maintained throughout.
2 · Targeted injection
The concentrated PRP is injected directly into the target tissue using precise anatomical technique. For erectile dysfunction treatment, this is an intracavernosal injection into the corpus cavernosum — the erectile tissue of the penis. Dr Bahl's 15-year specialisation in Urology provides the anatomical expertise required for accurate, safe delivery. The procedure takes approximately 20–30 minutes in total.
3 · Biological activation
Growth factors released from the concentrated platelets activate local repair mechanisms — stimulating neovascularisation (new blood vessel formation), nerve regeneration, collagen remodelling and tissue health at the injection site. When combined with prior focal shockwave therapy, the tissue has already been biologically primed to maximise response to the growth factor signal. Most patients require 1 to 3 sessions.
PRP Alone or Combined With Shockwave?
Both approaches are available at Shockwave Revibe Clinic. Dr Bahl will recommend the most appropriate protocol for your condition at your initial consultation.
| LI-ESWT Alone | PRP Alone | Combined | |
|---|---|---|---|
| Angiogenic stimulus | High (via VEGF) | Moderate | Very High |
| Growth factor delivery | Indirect (endogenous) | Direct (exogenous) | Synergistic |
| Evidence level | Multiple RCTs — EAU-endorsed | Early RCTs, case series | Emerging — limited RCT data |
| Best for | Vasculogenic ED, most cases | Adjunct or standalone | Complex ED, maximum response |

FIND OUT IF IT'S FOR YOU
Is PRP Therapy Right for You?
PRP therapy at Shockwave Revibe is used primarily for sexual health and erectile dysfunction. Dr Bahl will assess your suitability at an initial consultation — reviewing your medical history, current medications, and treatment goals before recommending a protocol.
Suitable for:
- Men experiencing erectile dysfunction (vasculogenic or mixed aetiology)
- Patients seeking enhancement of sexual function and tissue health
- Those who have completed a course of shockwave therapy and wish to consolidate results
- Men who have not responded adequately to PDE5 inhibitors (Viagra, Cialis)
- Patients seeking a non-surgical, drug-free alternative to ongoing medication
- Those interested in the combination LI-ESWT + PRP protocol for maximum biological effect
What to Expect (timeline):
-
1Consultation — Full clinical assessment with Dr Bahl. Review of medical history, medications, and suitability. Discussion of standalone vs combination protocol.
-
2Blood draw and preparation — 20–30ml blood sample processed on-site. Takes approximately 15–20 minutes.
-
3Injection — Intracavernosal injection using precise anatomical technique by Dr Bahl. 10–15 minutes.
-
4No significant downtime — Most patients resume normal activities the same day. Mild local discomfort possible for 24–48 hours.
-
5Sessions — Typically 1 to 3 sessions, 4–6 weeks apart depending on protocol and response.
-
6Results — Gradual improvement over 4–12 weeks as biological repair mechanisms activate.
CLINICAL EVIDENCE
What Does the Evidence Show?
PRP evidence for ED is at an earlier stage than shockwave therapy. Here is an honest summary of where the science currently stands.
| Evidence Area | Detail |
|---|---|
| PRP for ED — current evidence | Case series and early RCTs. Matz EL et al. (J Sex Med 2018) and Raveenthiran S et al. (Andrologia 2022) report improvements in IIEF-5 scores and patient satisfaction. Evidence level: emerging. |
| LI-ESWT for ED — established evidence | 14 RCTs, 833 patients. 60–75% of patients report meaningful improvement. EAU Guidelines 2024 endorse as evidence-based treatment for vasculogenic ED. (Zou ZJ et al., J Sex Med 2021) |
| Combination LI-ESWT + PRP | Biological rationale strong and compelling. Shockwave primes tissue; PRP amplifies growth factor signal. Limited dedicated combination RCT data — emerging. (Dr Bahl, Chapter 11) |
| Safety profile | PRP uses patient's own blood — no foreign substances, no rejection risk. Adverse events rare when prepared and delivered by a clinically trained specialist with appropriate anatomical technique. |
Important: PRP alone has early-stage evidence. LI-ESWT has strong RCT evidence. The combination has compelling biological rationale with emerging clinical data. Dr Bahl will give you an honest assessment at your consultation — not a sales pitch.
What we offer
PRP Shot Benefits
A natural, evidence-based approach using your body's own healing potential.
Improved Erection Quality
Patients typically experience firmer erections, increased sensitivity, better circulation and enhanced overall sexual performance within a few weeks.
100% Natural
PRP uses your own blood platelets — no foreign substances, no medication, no systemic side effects. A completely natural regenerative approach to sexual health.
Combines with Shockwave
PRP and shockwave therapy are often combined for enhanced and longer-lasting results in erectile dysfunction and Peyronie's disease.
PRP therapy represents a compelling biological adjunct to focal shockwave — your own growth factors, concentrated and delivered precisely where repair is needed. When sequenced correctly with shockwave, the synergy between the two is greater than either treatment alone.
